In order to become a full member of the club, the following forms must be filled out and given to an officer:

2011 Participation Agreement

A copy of your insurance card or proof of medical insurance must be attached to this form:

Vehicle Owner Form

After completely filling in the Vehicle Owner Form, you will need to print the form, sign it, and attach:

(1) Vehicle Registration

(2) Proof of Auto Insurance

(3) Driver's Licenses for All Authorized Drivers
