Liability Form

- Club participants must attach a copy of their medical insurance card or proof of insurance to the completed form. They may black out the policy number if they do not wish to disclose it. We will no longer require the participant to state his/her insurance company or write the policy # on the form but we must have a copy of the card/proof of insurance attached

-The participant must indicate the expiration date of their insurance policy on the participation agreement

-Participants will be required to state their affiliation to the club. They can check one of three boxes (Student, Faculty/Staff, or Community member). Students and faculty/staff must provide a PSU email address. Community members must provide their non-PSU email address if they have email.

Upon completion of the Liability Form, participants should print a copy and bring it along with a copy of thier medical insurance card or proof of insurance to practice and turn it in to a club officer!
