The first step in becoming part of the Penn State Hockey Organization is becoming a Penn State student. When completing your application, remember these guidelines:

Note: It is not necessary to complete the application prior to filling out the questionnaire below. However, we recommend that you fill out the admission forms in a timely manner.

Next, you will need to fill out our hockey questionnaire. We strongly urge you to review the players profile before continuing with the application. This form will ask for contact (address, phone, parent's/guardian's names), academic (High School GPA, SAT or ACT scores), and athletic (previous teams, stats) information.

Please try to provide as much information as possible. Also, be sure to review your answers thoroughly prior to submitting the form. The clicking below will launch a separate window with the form. Once you have completed and reviewed the form, you will be directed to a verification page.

Finally, we need your help to make Penn State an option for you in the future. If possible, please provide video highlights of your more recent games. Also, we ask that you include your jersey color and number in the video. With the volume of potential players coming in, this is a very important tool in helping us help you.
